web page access restriction request

make sure you've read the access restriction overview before submitting this. this page doesn't submit anywhere by itself — it builds an email to its-linux@binghamton.edu from what you enter below, and opens it in your normal mail client for you to review and send.

do not include your password anywhere on this form. generate your own credentials file with htpasswd and just give us the path to it. looking for a vhost account or a password reset instead? that's handled on the vhost account form.
must be a binghamton.edu address, non-BU addresses will be ignored
e.g. anthrojournal.binghamton.edu, or the vhost folder name under /local/htdocs
leave blank if you're only requesting host-based restriction
write this out using standard .htaccess syntax (AuthType/AuthName/AuthUserFile/Require, Order/Allow/Deny, etc.)
